The findings of this case are meaningful and the conclusions are important implications for clinical neurologist. In clinical, sudden hearing loss (SHL) and vertigo are both common symptoms. Usually, it's considered that SHL is mainly due to blood circulation disorder of the inthe internal auditory artery (IAA) which originating from the anterior inferior cerebellar artery(AICA) and in the absence of obvious stroke symptoms and excluding peripheral vertigo, disturbance of posterior circulation is the primary cause of vertigo. Brain imagings including CT and MRI are usually recommended to help find lesions, but not done in all clinical practice. The authors reported a case of SHL and vertigo as first symptoms that was finally diagnosed as cerebral infarction, and they suggest that SHL and vertigo could be the initial symptoms of vertebrobasilar ischemic stroke. It has implications for clinical practice, especially for the early identification of acute posterior circulation stroke. The cited references are recommended to be the latest, important and authoritative.
