This is an interesting case series of two patients with ankylosing spondylitis and positive HLA-B51, with negative HLA-B27, the latter being traditionally associated to the condition. While the authors are clear and concise in their case descriptions, grouping the case presentations together rather than detailing each section separately for each case would allow more clarity.
As mentioned by the authors, HLA-B51 is usually associated with Behcet's disease, and while the family members with signs of spondyloartropathy in this report shared the HLA-B51 allele, the high prevalence of this genetic marker in the region, along with the small n, make it very hard to conclude a definite association between the disease and the HLA-B51 allele. Furthermore, it is possible that other genetic makers that were not assessed in these patients may have contributed to the actual pathogenesis of this disease in this particular family.
