Hip displacement is a significant concern in children with cerebral palsy (CP), necessitating accurate and timely assessment to prevent long-term complications. This study developed a support vector machine (SVM) model to classify hip conditions using migration percentage (MP) measurements obtained from 176 hips across 88 anteroposterior pelvic radiographs. MP values were categorized into three groups: normal (MP ≤ 30 %), risky (30 % < MP ≤ 60 %), and dislocated (MP > 60 %). The SVM model was evaluated using stratified k-fold cross-validation, with accuracy, precision, recall, and F1-scores as key metrics. Its classifications were compared to manual evaluations performed by an orthopedic resident and a pediatric orthopedic surgeon. The model achieved an overall accuracy of 92.898 %, surpassing the consistency and reliability of manual assessments, particularly in identifying dislocated hips. Statistical analysis showed no significant differences between the model's MP measurements and those of the clinicians, validating its effectiveness. This study highlights the potential of SVM models to enhance diagnostic accuracy, reduce variability in evaluations, and support clinical decision-making. Future research should expand the dataset and incorporate advanced machine learning models to further improve diagnostic precision.

In recent years, the paradigm of surgical approaches for total hip arthroplasty (THA) has evolved, with portal-assisted techniques emerging as a promising avenue for increasing precision and minimizing invasiveness. The purpose of this study was to compare early experience with the Microport anterior percutaneously assisted total hip arthroplasty (MAP) system, with the established AMIS direct anterior approach (DAA).

A retrospective chart analysis was performed on 200 consecutive patients who underwent DAA or MAP at our institution in 2022. The research was conducted in accordance with the Declaration of Helsinki (as revised in 2013), and was approved by the institutional review board of the University Duisburg-Essen (23-11274-BO).

Two hundred patients were enrolled (100 DAA and 100 MAP; time to follow-up 1.7 years ±88 days). The mean operative time was 81 minutes (MAP) and 67 minutes (DAA, p>0.05). The mean cup tilt angle was 39° (MAP) and 40° (DAA; p>0.05). The mean cup anteversion angle was 13° (MAP) and 16° (DAA; p>0.05). The mean postoperative hemoglobin (Hb) decrease was 2.6 mg/dL ±0.9 mg/dL (MAP) and 2.5 mg/dL ±0.9 mg/dL (DAA; p>0.05). No major complications were documented in any of the 200 cases during the observation period. Additional screw fixation was performed in 7 cases and hybrid stem cementation was performed in 3 cases due to lack of rotational stability. All 10 cases were in patients with DAA. In only one of the 200 cases, two units of RBC were transfused postoperatively in a DAA case after a postoperative decrease of 5.7 mg/dL Hb.

Anterior Path® has been demonstrated to provide reliable results, despite the presence of a steep learning curve. The employment of a working cannula has been shown to enhance the surgeon's perspective during the preparation of the acetabulum. In relation to skin incision, the bikini line incision, which is regarded as advantageous due to its alignment with the cleavage lines, has been identified as a notable benefit that is acknowledged by the patient.

Maintaining appropriate soft tissue tension is essential in total hip arthroplasty (THA). Surgeons generally achieve this by altering the neck length. However, experimenting with different neck lengths is time-consuming, as it requires repeated hip dislocations. To address this, we have used a new extensible trial neck device that allows for easy adjustment of neck length during surgery. This device can help surgeons avoid the need for repeated hip dislocations to determine the correct neck size. The objective of this research was to investigate whether this device could help shorten operative times and decrease surgical invasion for patients.

Patients undergoing THA were randomly separated into two groups. The first group used the extensible trial neck during trial reduction after the stem and cup were placed (group M), while the second group used a conventional trial neck (group C). Operative time, blood loss, number of additional dislocations needed during the operation, and C-reactive protein (CRP) and creatine phosphokinase (CPK) levels after the operation were compared.

Operative time was significantly shorter and the number of additional dislocations required to choose the final neck size was significantly lower in group M compared with group C. No significant difference in blood loss was observed. CRP and CPK levels days 3 and 7 after surgery decreased in group M compared with group C.

The extensible trial neck was useful for THA by greatly reducing operative time and stress on the patient. [Orthopedics. 2025;48(2):e88-e93.].

Hip replacement (HA) is mainly indicated for the elderly, who generally suffer from various underlying diseases such as hypertension. This article provides a review of the key points of perioperative nursing care for patients with hypertension undergoing HA. It analyzes the key points of care during the perioperative period (preoperative, intraoperative, and postoperative) and proposes directions for the development of perioperative nursing care for HA. The prognosis for patients can be improved through the modification of traditional medical approaches and the application of new technologies and concepts.

Preventing dislocation with large head (≥ 36 mm), dual mobility, or constrained acetabular liner is another option than a standard (≤ 32 mm). Many other dislocations risk factors than size of the femoral head exist after hip arthroplasty revision. Predicting dislocation with a calculator according to the implant, to the indication of revision, and to patient's risks could allow a better surgery decision.

Our search method covers the period from 2000 to 2022. A total of 470 relevant citations on hip major revision (cup or stem or both revisions) were identified with artificial intelligence comprising 235 publications of 54,742 standard heads comprising 142 publications of 35,270 large heads, comprising 41 publications of 3945 constrained acetabular components, and 52 publications of 10,424 dual mobility implants. We considered four implant types (standard, large head, dual mobility, or constrained acetabular liner) as the entry layer of the artificial neural network (ANN). Indication for revision THA was the second hidden layer. Demographics, spine surgery, and neurologic disease were the third layer. Implant revision, reconstruction process as next input (hidden layer). Surgery-related factors, and so on. The output was a postoperative dislocation or not.

Of the 104,381 hips that underwent a major revision, a second revision for dislocation was performed for 9234 hips. In each implant group, dislocation remained the first cause of revision. The rate of second revision for dislocation as a percentage of first revision procedures was significantly higher in the standard head group (11.8%) than in the constrained acetabular liner group (4.5%), the dual mobility group (4.1%), and the large head group (6.1%). Instability of a previous THA, infection, or periprosthetic fracture as the indication for revision was increased risk factors as compared with aseptic loosening. One hundred variables were used to create the calculator with the best parameter combination of data and ranking the different factors, according to the four implant types (standard, large head, dual mobility, or constrained acetabular liner).

The calculator can be used as a tool to identify patients at risk for dislocation after hip arthroplasty revision and individualize recommendations to select another option than a standard head size.
