Abstract
This comprehensive systematic review and meta-analysis aimed to assess the impact of sesame (Sesamum indicum L.) supplementation on cardiovascular disease risk factors. Relevant research was discovered via PubMed, Scopus, Web of Science, CENTRAL, and EMBASE up to June 2024. The assessment of study quality was conducted using the Cochrane risk-of-bias tool. Thirteen trials, with interventions ranging from 4 to 12 weeks and involving 521 participants, demonstrated significant reductions in glycated hemoglobin (HbA1c) (Standardized Mean Difference [SMD] = - 0.67; 95% Confidence Interval [CI] - 1.01, - 0.32; P < 0.001), C-reactive protein (CRP) (SMD = - 0.51; 95% CI - 0.96, - 0.05; P = 0.028), and interleukin-6 (IL-6) (SMD = - 0.74; 95% CI - 1.16, - 0.32; P < 0.001), and a marginally significant effect on fasting blood sugar (FBS) (SMD = - 0.57; 95% CI - 1.16, 0.02; P = 0.057). Subgroup analyses revealed that sesame supplementation significantly reduced CRP and malondialdehyde (MDA) in populations without chronic diseases, while total cholesterol (TC) and MDA were reduced in those with chronic diseases. MDA was significantly reduced in females, especially those aged 50 or older. At dosages of 10 g per day or less, CRP, high-density lipoprotein cholesterol (HDL), and TC showed significant improvements. Meta-regression highlighted a significant dose-dependent reduction in TC levels at 10 g/day, and a significant duration-dependent decrease in TG levels at 8 weeks of supplementation. Sesame supplementation demonstrates potential benefits in improving glycemic control, inflammatory markers, and lipid profiles, making it a promising adjunct therapy for reducing cardiovascular disease risk factors.

Abstract
FADS1 rs174556 polymorphism influences on dietary fats metabolism and type 2 diabetes (T2DM). This study aimed to compare the effect of three oils of sesame, canola and sesame-canola on cardio metabolic factors across genotypes of rs174556 variant in patients with type 2 of diabetes. This study was a randomized triple-blind three-way cross-over clinical trial. 95 Subjects with T2DM replaced their regular dietary oil with sesame oil, canola oil, or sesame-canola oil for three 9-week phases and completed the study. There were three anthropometric measurements, blood sampling and biochemical assessments at the beginning, middle, and at the end of each phase for assessments. Genotyping was conducted using the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method. In the crude model, there was an interaction between consumed oils and rs174556 variant on serum concentration of Apolipoprotein A-I (ApoA-1). During intake of sesame oil, lower levels of triglycerides (TG) were observed in individuals with TT genotype compared to C allele carriers' allele, which remained significant in adjusted models. Compared to C allele carrier's, the people with TT genotype experienced significant increase and decrease in serum levels of HDL and TG, respectively in adjusted models. Also, the subjects who consumed sesame-canola oil had lower serum concentrations of fasting blood glucose than those who received sesame and canola oils, regardless of used oils and genotypes. FADS1 Gene variant (rs174556) might modify cardiometabolic changes following dietary vegetable oils. Larger longitudinal studies especially randomized clinical trials are needed to clarify these associations.

Abstract
BACKGROUND Obesity is a major public health issue with no definitive treatment. The first-line approach for obesity is lifestyle modification, including a healthy diet. Although the amount of fat has been considered, there is no network meta-analysis (NMA) study investigating the effect of edible oils on body weight. Therefore, we sought to investigate the effect of different edible oils on body weight using a systematic review and NMA study of randomized controlled trials (RCTs). METHOD PubMed, Scopus, ISI Web of Science, and the Cochrane Library were searched from inception to April 2019. RCTs of different edible oils for body weight were included. A frequentist network meta-analysis was conducted to appraise the efficacy of different types of edible oils, and the Surface Under the Cumulative Ranking Curve (SUCRA) was estimated. The GRADE framework was used to assess the certainty of evidence. RESULTS Forty-two eligible studies were included. Most of the included trials examined the effect of olive oil compared to canola oil (n = 7 studies), followed by canola oil compared to sunflower oil (n = 6 studies), and olive oil compared to sunflower oil (n = 4 studies). Sesame oil had the highest SUCRA value for reducing weight (SUCRA value = 0.9), followed by the mixture of canola and sesame oil (0.8). Palm oil and soy oil were ranked the lowest (SUCRA value = 0.2). CONCLUSION There is low to moderate certainty of evidence showing that soybean, palm, and sunflower oils were associated with weight gain, while sesame oil produced beneficial anti-obesity effects.

Abstract
AIMS The aim of this study was to conduct a systematic review and meta-analysis of randomized controlled trials (RCTs) that evaluated the impact of sesame supplementation on body weight (BW), body mass index (BMI), triglycerides (TGs), total cholesterol (TC), low-density lipoprotein-cholesterol (LDL-C), and high-density lipoprotein-cholesterol (HDL-C) in patients with type 2 diabetes mellitus (T2DM). DATA SYNTHESIS PubMed, Scopus, ISI Web of Science, and Embase were searched without any restrictions until September 2023.Only RCTs reporting the effects of sesame supplementation on body composition and lipid profiles were included, while observational studies and animal models were excluded. The methodological quality of the studies was assessed using the Cochrane risk of bias tool. Out of 997 studies identified, 10 were included in the systematic review and meta-analysis. Our meta-analysis suggested a significant association between sesame supplementation and reduction in TG (weighted mean difference (WMD): -37.61 mg/dl, 95 % CI: -61.48, 13.73), TC (WMD: -32.69 mg/dl, 95 % CI: -47.26, 18.12), and LDL-C (WMD: -28.72 mg/dl, 95 % CI: -44.68, 12.76). However, our meta-analysis indicated that the supplementary intake of sesame had no significant effect on HDL-C, BW, and BMI in patients with T2DM. CONCLUSIONS This study showed that sesame consumption significantly lowered TG, TC, and LDL-C levels, which may have contributed to the improvement of clinical symptoms in T2DM. However, given the limited number of trials included in the analysis, additional large-scale studies are needed to confirm the effects of sesame consumption on the lipid profile and body composition in patients with T2DM. Abstract
Sesame oil (SO) has been exhibited to have anti-inflammatory and antioxidant influences. The goal of this experiment was to look into SO's hepato-protective properties and underlying processes in valproic acid (VPA)-induced hepatotoxicity. Molecular docking was carried out to clarify the functional and structural underlying mechanism of SO ameliorative effect. Mice were given 8 mL/kg/day of SO (orally) and 100 mg/kg/day of VPA (i.p.) for 21 days. The results revealed that VPA caused a considerable increase in hepatic malondialdehyde levels while decreasing the activity of glutathione peroxidase (GPx) enzyme. There was also a significant rise in serum levels of interleukins 1β and 6 (IL-1β and IL-6) and a significant decrease in hepatic (PXR) gene expression level. SO co-administration with VPA significantly normalized the antioxidant and anti-inflammatory status and upregulated the gene expression level of PXR. In silico docking analysis results confirmed these results. This study concluded that supplementation of SO attenuated VPA-induced oxidative stress and inflammation. Hence, it was recommended as a dietary supplement for protection against VPA-induced hepatotoxicity.Communicated by Ramaswamy H. Sarma.

Abstract
Conflicting evidence exists on the effect of sesame consumption on glucose metabolism in patients with type 2 diabetes (T2D). Therefore, this meta-analysis focuses on the relationship between sesame (Sesamum indicum L.) intervention and glycemic control in patients with T2D. Published literature was retrieved and screened from PubMed, Scopus, ISI Web of Science, and the Cochrane Library up to December 2022. Outcome measures included fasting blood sugar (FBS) concentrations, fasting insulin levels, and hemoglobin A1c (HbA1c) percentage. Pooled effect sizes were reported as weighted mean differences (WMDs) and 95% confidence intervals (CIs). Eight clinical trials (395 participants) were eligible for meta-analyses. Overall, sesame consumption significantly reduced serum FBS (WMD: -28.61 mg/dL, 95% CI: -36.07 to -21.16, p˂0.001; I2 = 98.3%) and HbA1c percentage (WMD: -0.99%, 95% CI: -1.22 to -0.76, p ≤ 0.001; I2 = 65.1%) in patients with T2D. However, sesame consumption did not significantly influence fasting insulin levels (Hedges's: 2.29, 95% CI: -0.06 to 4.63, p = 0.06; I2 = 98.1%). In summary, the current meta-analysis showed a promising effect of sesame consumption on glycemic control through reducing FBS and HbA1c, yet additional prospective studies are recommended, using higher doses and longer intervention period, to confirm the impact of sesame consumption on insulin levels in T2D patients.

Abstract
Inflammation, oxidative stress, obesity, infection, hyperlipidemia, hypertension, and diabetes are the main causes of atherosclerosis, which in the long term lead to hardening of the arteries. In the current study, we reviewed recent findings of the mechanism of sesame and its active compounds of sesamin and sesamolin regulates on atherosclerosis. Sesame can decrease the lipid peroxidation and affect the enzymes, which control the balance of oxidative status in the body. Besides modulating the inflammatory cytokines, sesame regulates the main mediators of the signaling pathways in the process of inflammation, such as prostaglandin E2 (PGE2), nuclear factor kappa light-chain enhancer of activated B cells (NF-kB) and peroxisome proliferator-activated receptor gamma (PPAR-γ). Sesame decreases the growth of different pathogens. It fights against obesity and helps to reduce weight, body mass index (BMI), waist circumference, and lipid count of serum and liver. In addition to lowering fasting blood sugar (FBS), it decreases the hemoglobin A1c (HbA1c) and glucose levels and improves insulin function. With high content of linoleic acid, α-linolenic acid, and total polyunsaturated fatty acid (PUFA), sesame efficiently controls the blood plasma lipids and changes the lipid profile. In the case of hypertension, it maintains the health of endothelium through multiple mechanisms and conserves the response of the arteries to vasodilation. PUFA in sesame suppresses blood clotting and fibrinogen activity. All the mentioned properties combat atherosclerosis and hardening of blood vessels, which are detailed in the present review for sesame.

Abstract
Objective To evaluate safety and efficacy of dietary polyphenols in the treatment of rheumatoid arthritis (RA). Methods CNKI, Pubmed, Cochrane library, Embase were searched to collect randomized controlled trials (RCTs) of dietary polyphenols in the treatment of RA. The databases were searched from the time of their establishment to November 8nd, 2022. After 2 reviewers independently screened the literature, extracted data, and assessed the risk of bias of the included studies, Meta-analysis was performed using RevMan5.4 software. Results A total of 49 records (47 RCTs) were finally included, involving 3852 participants and 15 types of dietary polyphenols (Cinnamon extract, Cranberry extract, Crocus sativus L. extract, Curcumin, Garlic extract, Ginger extract, Hesperidin, Olive oil, Pomegranate extract, Puerarin, Quercetin, Resveratrol, Sesamin, Tea polyphenols, Total glucosides of paeony). Pomegranate extract, Resveratrol, Garlic extract, Puerarin, Hesperidin, Ginger extract, Cinnamon extract, Sesamin only involve in 1 RCT. Cranberry extract, Crocus sativus L. extract, Olive oil, Quercetin, Tea polyphenols involve in 2 RCTs. Total glucosides of paeony and Curcumin involve in more than 3 RCTs. These RCTs showed that these dietary polyphenols could improve disease activity score for 28 joints (DAS28), inflammation levels or oxidative stress levels in RA. The addition of dietary polyphenols did not increase adverse events. Conclusion Dietary polyphenols may improve DAS28, reduce C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR), and improve oxidative stress, etc. However, more RCTs are needed to verify or modify the efficacy and safety of dietary polyphenols. Abstract
Sesame (Sesamum indicum L.) is rich in polyunsaturated fatty acids, proteins, vitamin E, and lignans. Recent studies have highlighted the antioxidant, antihypertensive, hypolipidemic, and appetite-control properties of sesame seeds and sesame oil. However, there is a gap in the literature regarding the effect of tahini (sesame paste) consumption on human health. Thus, the aim is to investigate the postprandial effect of tahini consumption on blood pressure, endothelial function, and arterial stiffness. Twenty healthy men with mean age of 28 y and mean BMI of 25.81 kg/m2 were included. After a 12-h fast, baseline blood was collected, participants consumed 50 g of tahini, and blood collection was repeated 4 h postprandially. Assessment of blood pressure, pulse rate, hemodynamic parameters, and endothelial function was performed at baseline and at the end of the trial. Blood samples were used for the quantification of intercellular cell-adhesion molecule-1, vascular cell-adhesion molecule-1, and E-selectin levels at baseline and 4 h postprandially. A statistically significant decrease in diastolic blood pressure (p = 0.010) and pulse rate (p = 0.002) was observed 4 h after tahini consumption. Significant increases in serum triglycerides (p < 0.001) and flow-mediated dilatation were observed (p = 0.022) 4 h postprandially. No changes were observed in other indices measured at the end of the intervention compared with baseline. This is the first study to report that tahini consumption can lower blood pressure and pulse rate and improve endothelial function, suggesting a healthy snack in place of others with a less desirable lipid profile.

Abstract
BACKGROUND Non-alcoholic fatty liver disease (NAFLD) is one of the most common liver diseases in the world. There is strong evidence that dyslipidemia and other cardio-metabolic disorders are highly prevalent in patients with NAFLD. This trial aimed at examining the effect of sesame oil (SO) in the context of a weight loss program on lipid profile, blood pressure, and anthropometric indices in women with NAFLD. METHODS This randomized, double-blind, controlled trial was carried out on 60 women with NAFLD. Subjects were randomly assigned to the SO group (n = 30) and sunflower oil (SFO) group (n = 30), each person consuming 30 g of oil per day for 12 weeks. All the participants received a hypocaloric diet (- 500 kcal/day) during the study. Lipid profile, blood pressure, and anthropometric indices were assessed at pre- and post-intervention phases. RESULTS In total, 53 participants completed the study. Following 12 weeks of intervention, anthropometric indices (p < 0.001) and systolic blood pressure (SBP) (p < 0.05) were significantly decreased in both groups and diastolic blood pressure (DBP) was significantly decreased in So group (p = 0.03). There was no significant change in lipid profile in both groups (p > 0.05). After adjusting for confounders, DBP (p = 0.031) and total cholesterol (TC) divided by high-density lipoprotein cholesterol (HDL-C) (p = 0.039) in the SO group were significantly reduced compared to the SFO group (p < 0.05). CONCLUSIONS The present clinical trial revealed that SO and SFO may not differently affect anthropometric indices, SBP, and lipid profile except for TC/HDL-C. In addition, SO may be effective in improvement of DBP and TC/HDL-C compared to the SFO group. The effects of sesame, canola, and sesame-canola oils on cardiometabolic markers in patients with type 2 diabetes: a triple-blind three-way randomized crossover clinical trial. Abstract
AIMS To compare the effects of replacing regular dietary oils intake with sesame (SO), canola (CO), and sesame-canola (SCO) oils (a novel blend), on cardiometabolic markers in adults with type 2 diabetes mellitus (T2DM), in a triple-blind, three-way, randomized, crossover clinical trial. METHODS Participants were assigned to receive SO, CO, and SCO in three 9-week phases (4 weeks apart). Cardiometabolic makers (serum lipids, Apolipoprotein, cardiovascular risk scores, kidney markers, and blood pressure) were considered at the beginning and the end of intervention phases. RESULTS Ninety-two, ninety-five, and ninety-five participants completed the SO, SCO, and CO periods, respectively. After CO consumption, serum Apo A-1 concentrations were significantly higher compared with the SCO period in the whole population (p < 0.05). A considerable reduction in visceral adiposity index values was seen in the CO compared with the SO period in males (p < 0.05). Serum high-density lipoprotein concentration was also significantly higher after the SO intake compared with SCO in females (p < 0.05). The between-period analysis showed a substantial reduction in diastolic blood pressure in the SCO period compared with the CO and SO periods and lower systolic blood pressure after SCO versus CO intake in males (p < 0.05). CONCLUSIONS Canola oil might protect CVD through improving Apo A-1 levels in patients with T2DM (particularly in females) and visceral adiposity index in male patients. However, the blend oil might beneficially affect blood pressure in men. Future sex-specific studies might warrant the current findings. Abstract
SCOPE The aim of this study is to investigate the antidiabetic effect of lariciresinol (LSR) in C2C12 myotubes and streptozotocin (STZ)-induced diabetic mice. METHODS AND RESULTS To investigate antidiabetic potential of LSR, α-glucosidase inhibitory assay, molecular docking, glucose uptake assay, western blot assay on antidiabetic biomarkers are performed. STZ-induced diabetic model is used for in vivo study by calculating oral glucose tolerance test, histochemical examination, and glycogen assay. LSR inhibits α-glucosidase activity with an IC50 value of 6.97 ± 0.37 µM and acts as a competitive inhibitor with an inhibitory constant (Ki) value of 0.046 µM. In C2C12 cells, LSR activates insulin signaling leading to glucose transporter 4 (GLUT4) translocation and augmented glucose uptake. Furthermore, in Streptozotocin (STZ)-treated diabetic mice, 3 weeks of oral LSR administration (10 mg kg-1 ) considerably decrease blood glucose levels, while increasing insulin levels in an oral glucose tolerance test, improve pancreatic islet size, increase GLUT4 expression, and significantly enhance insulin signaling in skeletal muscle. LSR treatment also activates glycogen synthase kinase 3β (GSK-3β) resulting in improved glycogen content. CONCLUSION The findings indicate a potential usefulness for oral LSR in the management and prevention of diabetes by enhancing glucose homeostasis.

Abstract
Sesame, with an oily seed containing oil, lignans, and proteins, is a popular plant that has demonstrated health benefits such as antioxidative, antiobesity, and antiinflammatory effects. In this systematic review and meta-analysis, we aimed to summarize the effect of sesame seeds and their consumption compared to a control group on blood glucose and insulin resistance in human adults. PubMed/MEDLINE, SCOPUS, Web of Science, Google Scholar, and EMBASE were searched to identify eligible controlled clinical trials up to February 2021. Finally, eight clinical trials were included in this study. Sesame products used in these trials were sesame oil, sesamin, and tahini, and the duration of the intervention varied from 45 days to 9 weeks. Our results showed the significant positive effects of sesame and its products on fasting blood glucose FBG (weighted mean difference, WMD: -21.31 mg/dl, 95% CI: -41.23, -1.39, p = .036) and HbA1c (WMD: -0.75, 95% CI: -1.16, -0.34, p < .001) levels but results about fasting serum insulin (WMD: 5.51 μU/ml, 95% CI: -2.31, 13.33, p = .167) and HOMA-IR (WMD: -0.07, 95% CI: -0.33, 0.20, p = .617) were not meaningful. Sesame may be considered a beneficial agent for human glucose metabolism and can be a part of glucose-lowering diets.

Abstract
Background Clinical evidences showing the effects of sesame oil on metabolic biomarkers led to inconsistent results. Propose This meta-analysis was designed to examine the effects of sesame oil on metabolic biomarkers in adults, including the maximum number of clinical trials. Methods Google Scholar, PubMed, Web of Science, and Scopus were systematically searched to date up to July 2021 to identify eligible clinical trial studies. We obtained the pooled estimates of weighted mean differences (WMDs) with their 95% confidence intervals (CIs) using random-effects meta-analysis. Result Meta-analysis showed that sesame oil consumption significantly lowered the levels of fasting blood glucose (FBG) (WMD: -3.268 mg/dl; 95% CI: -4.677, -1.86; P < 0.001), and malondialdehyde (MDA) (WMD: -4.847 nmol/dL; 95% CI: -7.051, -2.698; P < 0.001) between the intervention and control groups. Also, this study showed sesame oil consumption significantly decreased HbA1C (WMD: -2.057%; 95% CI: -3.467, -0.646; P = 0.004), systolic blood pressure (SBP) (WMD: -2.679 mmHg; 95% CI: -5.257, -0.101; P < 0.001), diastolic blood pressure (DBP) (WMD: -1.981 mmHg; 95% CI: -3.916, -0.046; P = 0.045), body weight (WMD: -0.346 kg; 95% CI: -0.641, -0.051; P = 0.021), and body mass index (BMI) (WMD: -0.385 kg/m2; 95% CI:-0.721, -0.049; P = 0.025) after intervention. No significant effect was seen in serum insulin levels (p > .05). Conclusions The current study provided some evidence regarding the beneficial effects of sesame oil on metabolic biomarkers. Abstract
ETHNOPHARMACOLOGICAL RELEVANCE Sesamum indicum L. (Pedaliaceae) is an annual plant, which has been domesticated for well over 5000 years. It is widely cultivated for its seeds and is one of the oldest known oilseed crops. Traditionally, its seeds, seed oil, and different organs of the plant have been used to treat various diseases or conditions like ulcers, asthma, wound healing, amenorrhea, hemorrhoids, inflammations, etc. AIM OF THE REVIEW: The main aim of this review is to provide an outline and to assess the reported ethnopharmacological, phytochemical, pharmacological and toxicological studies of Sesamum indicum L. MATERIALS AND METHODS An extensive literature survey was done on various search engines like PubMed, Web of Science, Scopus, SciFinder, Google Scholar, Science direct, etc. Other literature sources like Wikipedia, Ethnobotanical books, Chapters were also studied to get maximum information possible on the Sesamum indicum L. RESULTS Over 160 different phytochemical compounds have been characterized and isolated from seeds, seed oil, and various plant organs, including lignans, polyphenols, phytosterols, phenols, anthraquinones, naphthoquinones, triterpenes, cerebroside, fatty acids, vitamins, proteins, essential amino acids, and sugars using suitable analytical techniques (e.g., LC-MS, GC-MS, HPTLC, HPLC). All the reported pharmacological activities like antioxidant, anticancer, antipyretic, antihypertensive, hepatoprotective, and anti-inflammatory are due to the virtue of these phytochemical compounds. CONCLUSION This review mainly highlights the botanical aspect of Sesamum indicum and its phytochemical constituents, ethnomedicinal uses, different pharmacological activities followed by ongoing clinical trials and future prospects. Sesamum indicum has great importance in traditional Indian medicine, which is further supported by modern pharmacological studies, especially in hepatoprotection, inflammation, and cancer. Several researchers have suggested that Sesamum indicum extracts and isolated compounds could have a wide therapeutic potency range. More research is needed to uncover key features of Sesamum indicum in medical practice, such as structure-activity relationships, toxicity, and therapeutic potential. In order to fully explore the plant's potential, safety assessments and implementation of an integrated cultivation method are also areas that need to investigate.

Abstract
BACKGROUND Recent investigations have proposed that sesame and canola oils might affect body fat distribution. The present study aimed to examine the effects of sesame, canola and sesame-canola (a blend of sesame and canola oils) oils on body weight and composition in adults with type 2 diabetes mellitus in the context of a randomized, triple-blind, three-way, cross-over clinical trial. RESULTS Eligible participants were randomized to replace their regular dietary oil with sesame oil (SO), canola oil (CO) and sesame-canola oil (SCO) (with 40% SO and 60% CO). Treatment periods lasted 9 weeks and were separated by 4-week wash-out periods. Body weight and composition were measured at the beginning, in the middle and at the end of each intervention phase. In total, 93 participants completed the study. After adjustment for confounders, within-period changes were observed following SO and CO intake for body weight (0.34 ± 0.16 kg and 0.33 ± 0.17 kg) and visceral fat (0.13 ± 0.06% and 0.13 ± 0.05%, P < 0.05), respectively. Body mass index was increased within SO intake (0.13 ± 0.05 kg m-2 , P = 0.031). All of the treatment oils resulted in reduced waist circumference and index of central obesity (P < 0.05). A significant difference in change values was observed for visceral fat between SCO (-0.14 ± 0.07%) and SO (0.12 ± 0.08%) treatment periods in females (P = 0.02). CONCLUSION Sesame and canola oils might lead to a modest favorable body fat redistribution by reducing central adiposity, particularly in females; however, the changes were of little clinical importance. © 2021 Society of Chemical Industry.

Abstract
OBJECTIVES In recent years, diabetes has become a global health problem that creates a tremendous economic burden for many countries. Clinical trials evaluating the hypoglycemic effects of sesame consumption have produced conflicting results. This systematic review and meta-analysis was conducted to evaluate the effectiveness of sesame as a popular natural herb on glycemic indices in adults. METHODS The search for related articles in PubMed, Scopus, Google Scholar, and Cochrane library was conducted through May 2021. Results were reported as weighted mean differences (WMD) with 95% confidence intervals (CI) using a random-effects model. RESULTS A total of 605 studies were identified through online searching, and a total of eight RCTs representing 382 participants were included in this study. The meta-analyses revealed that sesame consumption significantly decreases serum fasting blood sugar (FBS): (WMD: -28.23 mg/dl; 95% CI (-39.16, -17.13), I 2 = 97.6%; 95% CI (96, 98)), and hemoglobin A1c (HbA1c): (WMD: -1.00%; 95% CI (-1.11, -0.88), I 2 = 0%; 95% CI (0, 79)) as compared to the control group. CONCLUSION This study provides evidence of the hypoglycemic effects of sesame consumption, particularly in diabetic patients. Additional RCTs on sesame and its preparations should be conducted in different populations to increase generalizability.

Abstract
The present analysis was to summarize the evidence of the effects of sesame and its derivatives supplementation on cardiovascular disease (CVD) risk factors by performing a meta-analysis of randomized controlled trials (RCTs). Electronic databases were searched from their inception to July 2020. Two investigators independently assessed articles for inclusion, extracted data, and statistical analysis. The quality of included articles was assessed according to the Cochrane risk of bias tool. Major outcomes were synthesized using a random effect model and presented as weighted mean difference and 95% confidence interval. Heterogeneity, subgroup analyses, sensitivity analysis, meta-regression, and publication bias were also conducted. The GRADE approach was used to evaluate the quality of evidence. Overall, 16 trials involving 908 participants were included for statistical pooling. Compared with the control group, sesame intake significantly decreased the levels of total cholesterol, triglycerides, systolic blood pressure, diastolic blood pressure, body weight, body mass index, hip circumference, and waist circumference (P < 0.05). These results were stable in sensitivity analysis, and no significant publication bias was detected. Our findings provided evidence that sesame consumption may reduce the risk of CVD by improving blood lipids, blood pressure, and body weight management. Further large-scale, well-designed RCTs are required to confirm these results.

Abstract
The present study aimed to examine the effect of replacing edible oils with sesame oil (SO), canola oil (CO) and sesame-canola oil (SCO) on body weight and composition in adults. Adults without any chronic diseases (n = 77) were entered a 4-week run-in period and then were randomised to receive SO, CO and SCO for their household use in 9-week intervention periods (separated by 4-week washout intervals). Anthropometric measurements, as well as body composition markers, were assessed at baseline, middle and after each intervention period. In total, 73 participants completed the study. Although significant time effects were seen for waist and hip circumference, waist-to-hip ratio, central obesity index, body adiposity index, muscle mass and body fat percent (ptime<.05), the treatment and treatment × time effects were not significant (p>.05). The present clinical trial revealed that CO, SO and SCO might not differently affect body fat and composition. Trial registration code: IRCT2016091312571N6 (http://en.irct.ir/trial/12622).

Abstract
BACKGROUND Evidence suggests that the relations between intakes of individual fatty acids and risk of type 2 diabetes (T2D) vary. However, associations between intakes of different cooking oils as sources of fatty acids and incident T2D remain largely unknown. OBJECTIVES We aimed to evaluate relations between intakes of individual cooking oils and incident T2D in a nationwide Chinese cohort. METHODS Overall 15,022 Chinese adults aged ≥20 y from the China Health and Nutrition Survey (CHNS) without self-reported T2D at entry in the 1997, 2000, 2004, 2006, or 2009 rounds were followed up until 2011. Consumption of various cooking oils/fats including lard, peanut oil, soybean oil, canola oil, sesame oil, and refined blended plant oil was assessed using 3-d 24-h records in each survey and the cumulative mean intake was calculated. Multivariable-adjusted Cox proportional hazards regression models were constructed to estimate the HRs of T2D. RESULTS A total of 1014 cases were recorded after a median follow-up of 14 y. The intakes of animal and plant cooking oils/fats were both associated with higher T2D risk. Compared with nonconsumers, multivariable-adjusted HRs and 95% CIs for the highest tertiles were 1.31 (1.03, 1.67) for lard, 1.36 (1.10, 1.66) for peanut oil, 1.14 (0.91, 1.43) for soybean oil, 1.11 (0.87, 1.43) for canola oil, 1.02 (0.79, 1.32) for sesame oil, and 1.42 (1.12, 1.82) for refined blended plant oil. Substituting 1 tablespoon/d (8 g · 2000 kcal-1 · d-1) of soybean oil for the sum of lard, peanut oil, refined blended plant oil, and other plant oils was associated with a 3% (HR: 0.97; 95% CI: 0.95, 0.99) lower risk of T2D. CONCLUSIONS Intakes of lard, peanut oil, and refined blended plant oil but not soybean oil, canola oil, and sesame oil are associated with higher T2D risk. Reducing the consumption of cooking oils in general may be protective against T2D among the Chinese population.This trial was registered at clinicaltrials.gov as NCT03259321.

Abstract
BACKGROUND Both canola and sesame oils consumption have been associated with favorable effects on cardio-metabolic biomarkers. However, to the best of our knowledge, no study has compared their effects on cardiovascular risk factors. The present study aimed to assess the effect of canola, sesame, and sesame-canola oils consumption on cardio-metabolic biomarkers in patients with type 2 diabetes mellitus (T2DM). METHODS This study was a randomized, triple-blind, three-way, crossover clinical trial. The study participants included 102 individuals with T2DM. Their spouses were also included in the study. The participants were entered into a 4-week run-in period. After that, their regular dietary oil was replaced with canola, sesame, or sesame-canola oils (a blend of sesame and canola oils) in three 9-week phases, which were separated by two 4-week washout periods (sunflower oil was consumed during the run-in and the washout periods). Dietary, physical activity, blood pressure, and anthropometric measurements were assessed at the beginning, in the middle (week 4-5), and at the end of each treatment phase. Blood samples were taken at the beginning and at the end of each phase. Serum, plasma, buffy coat, and whole blood samples were extracted and kept at -80 ºC for further analysis. Serum fasting blood sugar (FBS), triglyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) were selected as the primary outcomes. RESULTS 102 participants with T2DM were randomly assigned to one of the 6 rolling methods. Through them, 93 individuals (91.2%) completely participated in all phases. CONCLUSION The present study will provide an exceptional opportunity to examine the effect of canola, sesame, and sesame-canola oil on cardio-metabolic markers in adults with and without T2DM. This trial will also provide a good medium for the investigation of gene-dietary oils interaction in the future.

Abstract
OBJECTIVES The study was designed to investigate the impact of white sesame seed oil (WSSO) consumption on fasting blood glucose (GLU), insulin (INS), glycosylated hemoglobin (HbA1c), and hepatic antioxidant enzymes. A secondary aim was to check the influence on serum biochemistry, hepatic, cardiac, and renal functions. METHODS Forty-six participants with type 2 diabetes were recruited and randomly divided into two equal groups: diabetic control (DCON) and diabetic sesame oil (DSO). At baseline and 30, 60, and 90 days, blood samples were drawn and analyzed. Two-way repeated-measures analysis of variance was used to evaluate the difference between groups and across time. RESULTS In both groups, GLU, INS, and HbA1c were not significantly different at baseline (mean 187.07 ± 5.63 mg/dl, mean 12.12 ± 1.03 μU/ml, and mean 7.55 ± 0.37%, respectively). At 90 days, GLU was significantly (p < 0.05) decreased in DSO (137.83 ± 3.16 mg/dl) when compared with DCON (218.13 ± 5.92 mg/dl), while INS was significantly increased in DSO (23.13 ± 1.15 μU/ml) as compared to DCON (7.93 ± 0.38 μU/ml). At 90 days, HbA1c was significantly lower (p < 0.05) in DSO as compared to DCON. Thiobarbituric acid reactive substances were significantly lower (p < 0.05) in DSO (1.08 ± 0.05 [MDA] nmol/ml) as compared to DCON (2.26 ± 0.07 [MDA] nmol/ml). In DSO, activities of hepatic antioxidant enzymes (superoxide dismutase, catalase, and glutathione peroxidase) increased while in DCON these activities decreased significantly (p < 0.05) across the time period. Biomarkers of liver, cardiac, and renal functions improved significantly in DSO as compared to DCON. CONCLUSION WSSO as a functional food may play an important role in GLU regulation and against deleterious effects of diabetes in humans with type 2 diabetes.

Abstract
We previously reported that sesame oil (SO) has anti-inflammatory, anti-atherosclerotic and lipid lowering properties in vivo. Our recent studies have shown that, an aqueous extract of sesame oil (SOAE) has also anti-inflammatory and anti-atherosclerotic properties but with no lipid lowering effects. The extent of reduction in atherosclerosis led us to identify components of SOAE and evaluate their anti-inflammatory properties in vitro. Liquid chromatography mass spectrometric method was used to detect and identify components of SOAE. Methoxyphenol derivatives, short and long chain carboxylic acids, dicarboxylic acids, hydroxy and oxo- carboxylic acids were detected. To our surprise, sesamol and its derivatives (lignans), were not present in the SOAE. Among the identified, a combination of methoxy phenol compounds were selected and tested their ability to reduce LPS induced inflammatory gene expression. Monocyte derived macrophages/RAW 264.7 macrophages were pre-treated with these compounds for 2 h, followed by LPS stimulation for 24 h and pro-inflammatory gene expressions were analyzed. These methoxyphenol derivatives showed potent anti-inflammatory properties. In conclusion, the anti-inflammatory molecules associated with SO may contribute the anti-inflammatory and anti-atherosclerotic properties. Also, our results shed light for the development of SOAE based non-pharmacological therapeutics, nutritional supplements and health products for various inflammatory diseases in the future.

Abstract
Hypertension is a major risk factor for cardiovascular disease, myocardial infarction, stroke and renal failure. Sesame consumption may benefit blood pressure (BP) owing to its high polyunsaturated fatty acid, fibre, phytosterol and lignan contents. To clarify this, a systematic review and meta-analysis of controlled trials was conducted. The PubMed (MEDLINE), Cumulative Index to Nursing and Allied Health Literature (CINAHL) and Cochrane Library (Central) databases were systematically searched until August 2016. Eight controlled trials with a total of 843 participants met the eligibility criteria. A random effect meta-analysis showed that sesame consumption can reduce systolic BP (-7.83 mmHg, 95% CI: -14.12, -1.54; P < 0.05, I2 = 99%) and diastolic BP (-5.83 mmHg, 95% CI: -9.58, -2.08; P < 0.01, I2 = 98%). To reduce the heterogeneity, the meta-analysis was limited to high methodology quality trials (n = 4), which resulted in a significant reduction in systolic BP (-3.23 mmHg, 95% CI: -5.67, -0.79; I2 = 33%) and a non-significant reduction in diastolic BP (-2.08 mmHg, 95% CI: -4.85, 0.69; I2 = 62%). This study concluded that sesame consumption can reduce systolic and diastolic BP. However, further investigations with larger sample sizes and better methodology quality are required to confirm the BP-lowering effect of sesame consumption. © 2017 Society of Chemical Industry.

Abstract
Diabetes is a complex condition with a variety of causes and pathophysiologies. The current single target approach has not provided ideal clinical outcomes for the treatment of the disease and its complications. Herbal medicine has been used for the management of various diseases such as diabetes over centuries. Many diabetic patients are known to use herbal medicines with antidiabetic properties in addition to their mainstream treatments, which may present both a benefit as well as potential risk to effective management of their disease. In this review we evaluate the clinical and experimental literature on herb-drug interactions in the treatment of diabetes. Pharmacokinetic and pharmacodynamic interactions between drugs and herbs are discussed, and some commonly used herbs which can interact with antidiabetic drugs summarised. Herb-drug interactions can be a double-edged sword presenting both risks (adverse drug events) and benefits (through enhancement). There is a general lack of data on herb-drug interactions. As such, more rigorous scientific research is urgently needed to guide clinical practice as well as to safeguard the wellbeing of diabetes patients.

Abstract
Sesame oil (SO) is a supplement that has been known to have anti-inflammatory and antioxidant properties, which makes it effective for reducing atherosclerosis and the risk of cardiovascular disease. Due to the side effects of statins, the current recommended treatment for atherosclerosis and cardiovascular diseases, the idea of using dietary and nutritional supplementation has been explored. The benefits of a dietary health regime have piqued curiosity because many different cultures have reaped health benefits through the ingredients in their cooking with negligible side effects. The purpose of this literary review is to provide a broad overview of the potential benefits and risks of SO on the development of atherosclerosis and its direction toward human clinical use. Current in vivo and in vitro research has shed light on the effects of SO and its research has shown that SO can decrease low-density lipoprotein (LDL) levels while maintaining high-density lipoprotein (HDL) levels. Current limitations in recent studies include no standardized doses of SO given to subjects and unknown specific mechanisms of the different components of SO. Future studies should explore possible synergistic and adverse effects of SO when combined with current recommended pharmaceutical therapies and other adjunct treatments.

Abstract
This study is aimed at assessing the scientific evidence on the effect of the intake of sesame seeds and derivatives on oxidative stress of individuals with systemic hypertension, dyslipidemia, and type 2 diabetes mellitus. A systematic review was conducted in seven databases (Lilacs, PubMed, ISI Web of Knowledge, Cochrane Library, Scopus, Trip Database, and Scielo) from September 2013 to January 2014. Clinical trials on the intake of sesame seeds and derivatives assessing the outcomes related to oxidative stress were retrieved. The risk of bias in the results of the studies selected was assessed according to the criteria of the Cochrane Handbook for Systematic Reviews of Interventions. This review included seven clinical trials showing that the intake of sesame resulted in the increase in enzymatic and nonenzymatic antioxidants, as well as in a reduction in oxidative stress markers. This was mainly observed with the use of sesame oil for hypertensive individuals during 2 months and black sesame meal capsules for prehypertensive individuals during four weeks. Most studies involved a small number of participants, sample size being considered a limiting factor for this review. In addition, a significant heterogeneity was observed in the type of population studied and the type of sesame and derivatives used, as well as their amount. The follow-up time was considered a limiting factor, because it varied in the different studies. The high risk of randomization and blinding biases found in the studies assessed determines lower scientific evidence of the results. Despite the limitations and biases identified in this systematic review, sesame showed relevant effects on oxidative stress, suggesting it could increase the antioxidant capacity.

Abstract
BACKGROUND Considering the health benefits of sesame oil and rice bran oil, the study was conducted to determine the extent to which the daily use of this blend of oils controls hyperglycemia and improves the lipid profile. METHODS In this 8-week open-label randomized dietary intervention study, 300 type 2 diabetes mellitus patients and 100 normoglycemic subjects were grouped as 1) normoglycemic subjects (n = 100) treated with sesame oil blend Vivo (Adani Wilmar, Ahmedabad, Gujarat, India), 2) type 2 diabetes mellitus patients treated with sesame oil blend (n = 100), 3) type 2 diabetes mellitus patients treated with glibenclamide (n = 100; 5 mg/d), and 4) type 2 diabetes mellitus patients treated in combination of glibenclamide (5 mg/d) and sesame oil blend (n = 100). Twelve-hour fasting blood glucose, glycated hemoglobin (HbA1c), and lipid profile followed by postprandial blood glucose were measured at baseline. Sesame oil blend was supplied to the respective groups, who were instructed to use as cooking oil for 8 weeks. Fasting and postprandial blood glucose was measured at week 4 and week 8, while HbA1c and lipid profile were measured at week 8. RESULTS At week 4 and week 8, type 2 diabetes mellitus patients treated with sesame oil blend or glibenclamide or combination of glibenclamide and sesame oil blend showed significant reduction of fasting and postprandial blood glucose (P <.001). HbA1c, total cholesterol, triglycerides, low-density lipoprotein cholesterol, and non-high-density lipoprotein cholesterol were significantly reduced (P <.001), while high-density lipoprotein cholesterol was significantly increased at week 8 (P <.001) in type 2 diabetes mellitus patients treated with the sesame oil blend or combination of glibenclamide and sesame oil blend; whereas glibenclamide-alone-treated type 2 diabetes mellitus patients showed a significant reduction of HbA1c (P <.001) only. CONCLUSIONS A novel blend of 20% cold-pressed unrefined sesame oil and 80% physically refined rice bran oil as cooking oil, lowered hyperglycemia and improved the lipid profile in type 2 diabetes mellitus patients.

Abstract
BACKGROUND Diabetes is one of the most common metabolic disorders and is related to oxidative-stress-induced diseases. Given the role of dietary antioxidants in the control and prevention of diabetes, this study aimed to examine the effects of sesame butter versus sesame oil on the serum levels of glucose, lipid profile, and oxidative stress biomarkers in diabetic rats. METHODS Forty male albino rats of Wistar strain were randomly divided into 4 groups (i.e., nondiabetic control rats, diabetic rats, diabetic rats treated with sesame butter, and diabetic rats treated with sesame oil). Experimental diabetes was induced with an intraperitoneal injection of streptozotocin (55 mg/kg). Sesame butter (1.25 g/kg) and sesame oil (0.5 g/kg) were given by oral gavage to the diabetic rats for 6 weeks. Finally, serum glucose, lipid profile, total antioxidant capacity (TAC), and malondialdehyde (MDA) levels were measured and analyzed statistically. RESULTS Our data showed that the diabetic groups treated with sesame butter and sesame oil had significantly lower levels of glucose and higher levels of high-density lipoprotein than did the diabetic control group at the end of the study (P<0.05). Sesame butter supplementation also increased TAC and decreased MDA concentrations significantly in the diabetic rats (P<0.05). CONCLUSION The antihyperglycemic, antioxidative, and partly lipid-lowering effects of sesame butter make it an excellent candidate for future human studies on diabetes, although further research is needed to determine the exact dose and duration of supplementation.

Abstract
AbstractIncreased plasma lipid profiles are among the most important risk factors of CHD and stroke. Sesame contains considerable amounts of vitamin E, MUFA, fibre and lignans, which are thought to be associated with its plasma lipid-lowering properties. This study aimed to systematically review the evidence and identify the effects of sesame consumption on blood lipid profiles using a meta-analysis of controlled trials. PubMed, CINAHL and Cochrane Library databases were searched (from 1960 to May 2015). A total of ten controlled trials were identified based on the eligibility criteria. Both the Cochrane Collaboration tool and the Rosendal scale were used to assess the risk of bias of the included studies. The meta-analysis results showed that consumption of sesame did not significantly change the concentrations of total blood cholesterol (−0·32 mmol/l; 95 % CI −0·75, 0·11; P=0·14, I2=96 %), LDL-cholesterol (−0·15 mmol/l; 95 % CI −0·50, 0·19; P=0·39, I2=96 %) or HDL-cholesterol (0·01 mmol/l; 95 % CI −0·00, 0·02; P=0·16, I2=0 %). However, a significant reduction was observed in serum TAG levels (−0·24 mmol/l; 95 % CI −0·32, −0·15; P<0·001, I2=84 %) after consumption of sesame. It was concluded that sesame consumption can significantly reduce blood TAG levels but there is insufficient evidence to support its hypocholesterolaemic effects. Further studies are required to determine the potential effect of sesame consumption on lipid profiles and cardiovascular risk factors.

Abstract
BACKGROUND Sesame oil and rice bran oil are known for their unsaturated fatty acids and antioxidants contents and have been reported to reduce the cardiovascular risk. OBJECTIVE To determine the effect of a blend of 20% unrefined cold-pressed lignans-rich sesame oil and 80% physically refined γ-oryzanol-rich rice bran oil (Vivo) as cooking oil in mild-to-moderate hypertensive patients. METHODS In this prospective, open-label dietary approach, 300 hypertensive patients and 100 normotensives were divided into groups as: (1) normotensives treated with sesame oil blend, (2) hypertensives treated with sesame oil blend, (3) hypertensives treated with nifedipine, a calcium channel blocker (20 mg/d), and (4) hypertensives receiving the combination of sesame oil blend and nifedipine (20 mg/d). Sesame oil blend was supplied to respective groups, and they were instructed to use it as the only cooking oil for 60 days. Resting blood pressure was measured at days 0, 15, 30, 45, and 60, whereas the fasting lipid profile was measured at days 0 and 60. RESULTS Significant reduction in blood pressure (systolic, diastolic, and mean arterial) from days 0 to 15, 30, 45, and 60 were observed in hypertensives treated with sesame oil blend alone (P < .001), nifedipine alone (P < .001), and combination of sesame oil blend and nifedipine (P < .001). Sesame oil blend with nifedipine-treated group showed greatest reduction in blood pressure. Total cholesterol, low-density lipoprotein cholesterol, triglycerides, and non-high-density lipoprotein cholesterol levels reduced, whereas high-density lipoprotein cholesterol levels increased significantly only in hypertensives treated with sesame oil blend alone and the combination of sesame oil blend and nifedipine (P < .001). CONCLUSION We demonstrate for the first time that using a blend of sesame oil and rice bran oil as cooking oil showed a significant antihypertensive and lipid-lowering action and had noteworthy additive effect with antihypertensive medication.

Abstract
Atherosclerosis, a major form of cardiovascular disease, has now been recognized as a chronic inflammatory disease. Nonpharmacological means of treating chronic diseases have gained attention recently. We previously reported that sesame oil has anti-atherosclerotic properties. In this study, we have determined the mechanisms by which sesame oil might modulate atherosclerosis by identifying genes and inflammatory markers. Low-density lipoprotein receptor knockout (LDLR(-/-)) female mice were fed with either an atherogenic diet or an atherogenic diet reformulated with sesame oil (sesame oil diet). Plasma lipids and atherosclerotic lesions were quantified after 3 months of feeding. Plasma samples were used for cytokine analysis. RNA was extracted from the liver tissue and used for global gene arrays. The sesame oil diet significantly reduced atherosclerotic lesions, plasma cholesterol, triglyceride, and LDL cholesterol levels in LDLR(-/-) mice. Plasma inflammatory cytokines, such as MCP-1, RANTES, IL-1α, IL-6, and CXCL-16, were significantly reduced, demonstrating an anti-inflammatory property of sesame oil. Gene array analysis showed that sesame oil induced many genes, including ABCA1, ABCA2, APOE, LCAT, and CYP7A1, which are involved in cholesterol metabolism and reverse cholesterol transport. In conclusion, our studies suggest that a sesame oil-enriched diet could be an effective nonpharmacological treatment for atherosclerosis by controlling inflammation and regulating lipid metabolism.

Abstract
BACKGROUND This study was designed to assess the effect of sesame seed on lipid profile and oxidative stress biomarkers in knee osteoarthritis (OA) patients. METHODS Fifty patients with knee OA were allocated into two groups receiving 40 g of sesame seed daily along with standard medical therapy (n=25) or stan-dard treatment (n=25) for two months. Serum total antioxidant capacity, ma-londialdehyde (MDA) and lipid profile (total cholesterol (TC), HDL-cholesterol, LDL-cholesterol, triglycerides) were measured. RESULTS After the intervention period two months of study, serum TC, LDL-cholesterol and MDA decreased significantly in the sesame group (P<0.05), while no significant difference in serum values of lipid profile and oxidative stress parameters was seen in the control group (P>0.05). There was no signifi-cant difference in pre and post-treatment values of lipid profile and oxidative parameters between the two groups (P>0.05). CONCLUSION Current study showed a positive effect of sesame seed in improv-ing lipid profile and oxidative stress in patients with knee OA and indicated the fact that sesame seed might be of help to reduce oxidative stress in OA patients.

Macronutrients and micronutrients can prevent, control and treat hypertension through numerous mechanisms related to vascular biology. Oxidative stress, inflammation and autoimmune dysfunction initiate and propagate hypertension and cardiovascular disease. There is a role for the selected use of single and component nutraceutical supplements, vitamins, antioxidants and minerals in the treatment of hypertension based on scientifically controlled studies which complement optimal nutrition, coupled with other lifestyle modifications.

Abstract
The possible effects of sesame oil on hemodynamics are unknown. The aim of the study was to investigate the acute and long-term effects of sesame oil on hemodynamic responses in hypertensive men. The authors enrolled 30 hypertensive men in a two-phase study. In the first phase, patients consumed 35 g of either sesame oil or control oil. Central and peripheral blood pressure, pulse wave velocity, augmentation index (AI), C-reactive protein, tumor necrosis factor α, malonydealdehyde, and total antioxidant capacity (TAC) were assessed at fast and 2 hours postprandially. In the second phase, patients consumed 35 g of either sesame oil or control oil daily for 2 months. The above-mentioned parameters were assessed before and following 15, 30, and 60 days of oil consumption. Sesame oil decreased central and peripheral diastolic pressures 1 hour postprandially (P=.006). Fifteen days of sesame oil intake decreased peripheral systolic blood pressure (P=.016) and heart rate-corrected AI75 (P=.017) and increased TAC (P=.007). This is the first study to demonstrate a favorable acute and long-term effect of sesame oil on hemodynamics in hypertensive men. Further research is warranted to establish the potential protective role of sesame oil.

Abstract
BACKGROUND The aim of the study was to investigate the effects of sesame oil on endothelial function and to detect the underlying mechanisms, both in the postprandial state and after long-term consumption. DESIGN We enrolled 30 hypertensive men in a two-phase study. In the first phase, 26 volunteers consumed 35 g of either sesame oil or control oil. Endothelial function, inflammatory activation and nitric oxide syntase (NOS) inhibition was assessed after a 12-hour fast and 2 hours after consumption of an oil-containing standardized meal. In the second phase, 30 volunteers consumed 35 g of sesame oil or control oil daily for 2 months and the above-mentioned parameters were assessed at baseline, 15, 30 and 60 days. METHODS Endothelial function was estimated by endothelium-dependent FMD (flow-mediated dilatation) of the brachial artery. RESULTS Flow-mediated dilatation (FMD) improved significantly both after acute (p = 0.001) and long-term sesame oil consumption (p = 0.015, p = 0.005 and p = 0.011 for 15, 30 and 60 days respectively). Intracellular adhesion molecule (ICAM) levels decreased significantly after only 60 days of daily sesame oil intake (p = 0.014). By contrast, no changes were observed in the control group in either phase of the study. CONCLUSIONS This is the first study to show that sesame oil consumption exerts a beneficial effect on endothelial function and this effect is sustained with long-term daily use.

Abstract
BACKGROUND & AIMS Recently, studies have reported that sesame oil lowered blood pressure and improved antioxidant status in hypertensive and diabetic-hypertensive patients. The aim of this study was to evaluate the effectiveness of sesame oil with anti-diabetic (glibenclamide) medication as combination therapy in mild-to moderate diabetic patients. METHODS This open label study included sixty type 2 diabetes mellitus patients divided into 3 groups, receiving sesame oil (n = 18), 5 mg/day (single dose) of glibenclamide (n = 20), or their combination (n = 22). The patients were supplied with sesame oil [BNB Sesame oil(TM)] except glibenclamide group, and instructed to use approximately 35 g of oil/day/person for cooking, or salad preparation for 60 days. 12 h-fasting venous blood samples were collected at baseline (0 day) and after 60 days of the experiment for various biochemical analysis. RESULTS As compared with sesame oil and glibenclamide alone, combination therapy showed an improved anti-hyperglycemic effect with 36% reduction of glucose (P < 0.001 vs before treatment, P < 0.01 vs sesame oil monotherapy, P < 0.05 vs glibenclamide monotherapy) and 43% reduction of HbA(1c) (P < 0.001 vs before treatment, P < 0.01 vs sesame oil monotherapy, P < 0.05 vs glibenclamide monotherapy) at the end point. Significant reductions in the plasma TC, LDL-C and TG levels were noted in sesame oil (20%, 33.8% and 14% respectively vs before treatment) or combination therapies (22%, 38% and 15% respectively vs before treatment). Plasma HDL-C was significantly improved in sesame oil (15.7% vs before treatment) or combination therapies (17% before treatment). Significant (P < 0.001) improvement was observed in the activities of enzymatic and non-enzymatic antioxidants in patients treated with sesame oil and its combination with glibenclamide. CONCLUSIONS Sesame oil exhibited synergistic effect with glibenclamide and can provide a safe and effective option for the drug combination that may be very useful in clinical practice for the effective improvement of hyperglycemia.
