"Level effect" meant that the estimated coefficients reflect the marginal effects of the explanatory variables on the levels of total transition scores. Column 4-5 showed results from the second regression, where we included the lagged transition score as an additional independent variable and Sample 3 was used for estimation. "Growth effect" meant that conditional on the lagged transition score, the coefficients in the regression reflect the marginal effects of the explanatory variables on the growth in transition score between two scores. We estimated robust standard errors clustered at the patient level for both regressions.
