Alcohol consumption ＞ 40 mL per day

Diabetes or impaired glucose tolerance

(including hyperinsulinaemia, OGTT and fasting blood glucose)

Under- or overweight (BMI ＜ 18 or ＞ 26 kg/m2)

Dyslipidaemia

Hyper- or hypothyroidism

Gluten enteropathy

Autoimmune hepatitis or PBC/PSH

Viral hepatitis B or C

Hereditary hemochromatosis or Iron overload

Wilson disease

Anamnesis for hepatotoxic drugs use (including herbal and over-the-counters in the last 12 mo)

Hereditary muscular disorders
