| Reduced awareness of hypoglycemic episodes and/or severe glycemic lability despite intensive insulin therapy and glycemic monitoring | 
| Exclusion criteria | 
| Presence of uncorrectable coronary disease | 
| A body mass index of > 25 kg/m2 | 
| Inadequate renal reserve defined as serum creatinine level of > 1.5 mg/dL, creatinine clearance of < 80 mL/min per 1.73 m2 of body surface area, or a urinary albumin level of > 300 mg/24 h | 
| Negative serological findings for Epstein-Barr virus at the time of assessment |
