This manuscript aimed to review the current literature to assess the safety of EOS surgical treatment in terms of the rate of complications and unplanned surgeries. In January 2021 two independent reviewers systematically searched three electronic medical databases (PubMed, the Cochrane Library, and Embase) for relevant articles. Every step of the review was done according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Due to the heterogeneity of articles and topics after data analysis, a descriptive (synthetic) analysis was performed.
I read the article with interest, the title is well thought out and faithfully reflects the content of the study.
The abstract is adequately developed, and it is useful to frame the purpose of the study. In the introduction, the characteristics of the Early-onset scoliosis have been described. In materials and methods is adequately developed. The use of tables are adequate for better understanding. The discussion is sufficiently developed.
Comment 1: In the abstract: the purpose of the study should be specified in order to clarify the characteristics of the study to the reader.
Comment 2: In the introduction: It would be better to elaborate on the aspects on the pathogenesis of the Early-onset scoliosis Early-onset scoliosis, adding appropriate bibliographical references. For example: (Karol LA. (2019) "The Natural History of Early-onset Scoliosis" or Yang S, et al (2016) "Early-Onset Scoliosis: A Review of History, Current Treatment, and Future Directions. Pediatrics").
Comment 3: In the introduction: "As such, EOS includes spinal deformities resulting from congenital malformations, neuromuscular conditions, inherited bone dysplasias and syndromes, and, in idiopathic cases, with no underlying disorder." Can you improve this aspect, adding appropriate bibliographical references.
Comment 4: In materials e method: Please specify the experience of the three investigators independently reviewed, are they orthopedist surgeon?
Comment 5: In the results: Were the exclusion criteria for moving from 1300 articles to 615 and subsequently from 615 to 40 articles the same? This step seems unclear
Comment 6: In the results: In the studies analyzed how many of the patients were female and how many were male? what was the average age? How many operators were there who performed the surgical treatments?
Comment 7: In the results: Were the complications treated by the same surgeons who had performed the previous treatments?
Comment 8: In the discussion: It's not very clear the limitations of your study and what should be done in future studies to improve these limitations.
